_____Acute loss of smell or taste
_____Loss of appetite
_____Unusual fatigue/lack of energy
_____In the last 14 days have you been in close contact with a known case of COVID-19?
_____In the last 14 days, did you have close contact with an ill person who travelled outside of Newfoundland & Labrador?
_____Have you received a notification from the COVID Alert app that you may have been exposed to COVID-19?
_____Have you tested positive for Covid-19
_____Have you travelled outside Newfoundland in the last 14 days?
_____Have you been asked to self monitor, isolate or quarantine yourself in the last 14 days?
_____Have you received a Covid-19 vaccine in the last 2 weeks or plan to receive one in the next 2 weeks
There is no cancellation fee. Please don’t come if you are feeling unwell.
1-Please text me to notify me of your arrival. You can wait in your car or come to the waiting room and physically distance.
2-When I am ready to see you, I will text you or come get you in the waiting room.
3-Virtual or telephone consults are available. No in office consults at this time. Please come only if you intend to be treated. Please be precise as to what treatment you require, so I can estimate the time required.
4-All patients are asked to remove makeup from the area being treated prior to entering the clinic and to wear a mask or facial covering (nose & mouth) into the office . I can provide one for you if you do not have one. This must be worn at all times except when being treated.
5-You may attend your appointment with one other person, as long as they are in your bubble.
6-Please leave all non-essential items in your vehicle including jackets, hats, books, bags, food & beverages.
7-Please sanitize your hands upon arrival using the hand sanitizer provided outside my treatment room door.

Please note: Laser or IPL services are not available at this time. All appointments are for injectables only.
Current epidemiologic information suggests that human-to-human transmission of COVID-19 can occur when an individual is in close contact with a positive Covid case, contact with a traveller and from unknown cases.
Human coronaviruses are most commonly spread from an infected person through: respiratory droplets; close, prolonged personal contact; and touching an infected area, then touching mouth, nose or eyes before washing hands.
However, given the nature of the treatments being administered, you are assuming the risk of acquiring this virus from asymptomatic patients that may have entered the building leaving the virus without their knowledge.
The disinfecting, wearing of masks, skin preparation, hand sanitizing , social distancing by staying in your car and pre screening may not prevent the transmission.
You will be asked to sign a consent assuming this risk for this elective treatment.
